ADHD Self-Assessment for Adults

 

This questionnaire is adapted from the World Health Organization’s Adult ADHD Self-Report Scale (ASRS v1.1), developed in collaboration with Harvard Medical School. It is one of the most widely used screening tools for ADHD in adults.

It includes 18 questions that reflect the diagnostic criteria for ADHD in the DSM-5. The first 6 questions (Part A) are the most predictive. Part B provides additional context.

This tool is designed for screening purposes only. It does not provide a diagnosis. Only a qualified clinician can make a formal diagnosis of ADHD.
